Medical Records

You may request medical records to be prepared by simply contacting our medical records department. Please fill out the form below and return to West Georgia Cardiology in person, or via fax or email. Upon receipt of your request, one of our staff members will prepare your records and ensure that they are ready for pickup or fax.  Please allow up to 4 weeks for processing.
